Sometimes, and it depends on the sequence. If the work can be phased so a bathroom and a kitchen or kitchenette stay usable, staying is realistic. If mechanical or electrical work takes the whole house down at once, it is not. Settle this before demolition, because it changes both the sequence and the price.
It can. Once walls are open, work that is exposed generally has to meet current code even if it was compliant when installed. That most often affects wiring, and sometimes plumbing and insulation. It is not a penalty, it is the point at which those upgrades become affordable, but it needs to be in the budget from the start.
Ask before you sign. The answer should be that anything outside the written scope is quoted and approved by you before the work continues, in writing. A contractor who absorbs surprises silently is either padding the original number or will come back for it later.
